New passport applications are accepted now at Customer Service by appointment only with one business day's notice.  Appointments are available Monday through Friday in thirty minute increments to allow for adequate processing time for families.

Customers may fill out their passport applications online via the U.S. Department of State  www.travel.state.gov. Hard copy applications are also available at our office. Customers must have all documentation complete and available upon arrival for processing. 

-Required Documents for first time applicants:

  • Application (DS-11)
    • Completed in BLACK INK
    • Signed in Agent's presence after oath
  • Driver's license or other government issued photo I.D.
  • Certified birth certificate and/or current/expired passport
  • 1 color passport photo
  • Check or money order to process with each application

-Required documents for passport renewals:

  • Application (DS-82) Application for a U.S. Passport by Mail
  • Current or Expired certified Passport (cannot be issued more than 15 years prior to submission)
    • Customers do not submit these to a passport acceptance facility (City of Sheridan).
    • Renewals must be mailed with payment directly to the address listed on the application. 

-Required documents for minors under the age of 16:

  • The driver's license (or other government issued photo I.D.) of each parent
  • Certified birth certificate and expired passport, if you have one
  • 1 color passport photo
  • Minor MUST appear in person with parents

-Federal law requires the following payments for passport processing fees:

  • A $35 passport acceptance fee payable to the City of Sheridan, in the form of cash, check, money order, or credit card.
    • Fee is assessed per passport processed.
  • One check or money order for each application, and any other Department of State fees applicable.
